What NAD+ is

NAD+ — nicotinamide adenine dinucleotide — is a coenzyme present in every cell in your body. It is central to how cells convert food into usable energy, and it is a required substrate for several repair enzymes, including the sirtuins and PARPs involved in DNA repair and cellular maintenance.

NAD+ levels decline with age. That is well documented in laboratory research, and in animal models restoring NAD+ produces measurable improvements in mitochondrial function and markers associated with ageing. This is real science from serious laboratories, and it is why the topic gets the attention it does.

The open question — the one the marketing skips — is whether infusing NAD+ intravenously in humans produces meaningful clinical benefit. That has not been established, and we will come back to it.

What a session actually involves

This is the part most clinics underplay, and you should know it before you book.

NAD+ infusions are slow, and they are not comfortable. Infused too quickly, NAD+ commonly produces chest tightness, abdominal cramping, nausea, flushing and a general sense of unease. It is not dangerous, but it is unpleasant, and the only way to manage it is to slow the drip right down.

That means a full NAD+ infusion typically takes two to four hours, sometimes longer for a first session. Anyone offering you a quick NAD+ drip on a lunch break is either using a very small dose or is going to make you feel unwell.

  1. Evaluation first. History, current medications, and in most cases recent labs.
  2. Slow start. We begin at a low rate and increase only as you tolerate it.
  3. You tell us how you feel. The rate is adjusted throughout based on your symptoms. This is a collaborative infusion, not a set-and-forget one.
  4. Monitoring. Clinical staff supervise you throughout.
  5. Afterward. Most people feel fine and go about their day. Some feel tired.

Bring something to do. You will be there a while.

What we use it for

People come to us about NAD+ for chronic fatigue, brain fog and cognitive complaints, post-viral illness including Long COVID, recovery from prolonged stress or burnout, and general healthy-ageing interest.

Whether NAD+ is the right answer for any of those depends on what is actually driving the fatigue — and fatigue has a long list of causes worth ruling out first, including thyroid dysfunction, anaemia, sleep apnoea, mold and biotoxin illness, Epstein-Barr reactivation and depression. An expensive infusion for undiagnosed sleep apnoea helps nobody.

What the research currently shows

We want to be straightforward, because there is a great deal of confident marketing around NAD+.

The preclinical evidence is strong and genuinely interesting. NAD+ decline with age is well documented, and in animal models restoring NAD+ improves mitochondrial function, metabolic markers and several measures associated with ageing.

The human evidence is much more limited. Most clinical research to date has used oral NAD+ precursors — nicotinamide riboside and nicotinamide mononucleotide — rather than intravenous NAD+. Those trials generally show that blood NAD+ levels can be raised safely, but results on clinical outcomes such as energy, cognition and physical function have been modest and inconsistent. Trials of intravenous NAD+ specifically are few and small. One pharmacokinetic study demonstrated that IV NAD+ does reach the bloodstream and is metabolised, which tells us it gets in — not that it produces benefit.

There is a longer history of IV NAD+ use in addiction and withdrawal settings, but that too rests largely on clinical reports rather than controlled trials.

NAD+ IV therapy is not FDA-approved for the treatment of any condition. If a clinic tells you NAD+ will reverse ageing, repair your DNA, or resolve chronic fatigue, they are describing something the evidence does not currently establish. We are not going to tell you that.
