What ozonated clot matrix therapy is

Ozonated clot matrix is a self-derived injection therapy. We draw a small volume of your blood, process it to concentrate the platelets and the fibrin they sit in, and produce a clot matrix — a stable, gel-like scaffold rather than a thin liquid.

That structural difference is the point. A plasma preparation disperses fairly quickly after injection. A fibrin matrix holds its shape at the site, which allows the growth factors held inside it to be released gradually rather than all at once.

Before injection we combine that matrix with medical ozone, collagen, and targeted nutrients. The ozone and the collagen each do a different job, and we explain both below.

Everything structural in the preparation comes from you. Nothing is harvested from a donor, and no living cells from any other source are introduced.

Why we add ozone rather than using it on its own

This is the part worth reading carefully, because it is where the published research is genuinely informative — and where a lot of clinics overstate.

A meta-analysis of 12 randomized controlled trials covering roughly 970 knee osteoarthritis patients compared oxygen-ozone therapy and platelet preparations head to head. The finding on ozone used alone was sobering: a mean visual analogue scale difference of just −0.02 against controls in the first month, with improvements fading by six months. Platelet preparations, by contrast, produced sustained and statistically significant gains — pain reduced by 3.53 points, stiffness by 0.60, and physical function by 5.96, holding across six to twelve months. We take that seriously. It is why we do not offer ozone alone as a joint injection. The evidence does not support it as a standalone answer for a worn joint, and we would rather say so than sell it.

What the ozone appears to contribute is something narrower and genuinely useful. A study comparing a combined ozone-and-platelet preparation against a platelet preparation alone in early knee osteoarthritis found that at three months the combined group had significantly better physical function and total WOMAC scores. It also found lower pain scores at day 10 and significantly less hyper-inflammation at the injection site. The authors concluded that patients receiving ozone alongside the platelet preparation were less likely to experience post-injection pain and more likely to recover faster. Source

That was a retrospective comparison of 80 patients rather than a randomized trial, so it carries less weight than the meta-analysis above. But the direction is consistent and the mechanism is plausible: the matrix does the regenerative work, and the ozone makes the days immediately afterwards easier.

What the collagen and nutrients are for

Collagen is added as a structural component of the scaffold rather than as a treatment in its own right. The nutrient additions support the local environment the matrix is placed into. Neither is offered as an independent therapy, and we make no separate efficacy claim for either.

If you want the honest summary: the platelet and fibrin content is the part with the research behind it, the ozone appears to improve the recovery experience, and the remaining components support the preparation itself.

How this differs from PRP

They are related but not the same, and the distinction is worth understanding before you choose.

  • Ozonated clot matrix is a fibrin-based scaffold rather than a liquid, combined with ozone, collagen and nutrients before injection. It is performed by Alvin Philipose, DC, ICCP.

Where the two have been compared directly. In the one head-to-head comparison available — a retrospective study of 80 patients with early knee osteoarthritis — adding ozone to a platelet preparation produced significantly better physical function and total WOMAC scores at three months, along with lower pain at day 10 and faster recovery, than the platelet preparation alone. One retrospective study of 80 patients is not proof, and we will not present it as such. It is the direction the available evidence points, and it is part of why we prepare it this way.

Which one suits you depends on the joint, the pattern of your pain, and what you have already tried. Part of the evaluation is working that out rather than defaulting to whichever is more fashionable.

Conditions we use it for

  • Knee pain and osteoarthritis
  • Shoulder, hip and ankle joint pain
  • Chronic tendon and ligament problems that have not settled with rest or physical therapy
  • Joint instability, often alongside prolotherapy or prolozone
  • Low back and sacroiliac pain of ligamentous origin

Where the pain pattern points to cartilage wear rather than laxity, we will often discuss correctly dosed photobiomodulation as well, which has the deepest randomized evidence base of anything we offer for knees.

How a session works

  1. Evaluation. History, examination, and review of any imaging you already have. We establish what kind of problem this is before deciding whether the matrix is the right tool.
  2. Blood draw. A small volume, taken the same day.
  3. Preparation. Your blood is processed on site into a fibrin matrix and combined with ozone, collagen and nutrients.
  4. Injection. Placed into the target joint or soft tissue, guided by examination findings.
  5. Recovery. Most people are sore for a few days. The research above suggests the ozone component makes this stretch easier than a plasma injection alone.
  6. Reassessment. We measure against the baseline we took at the start, so “did this help” gets answered with something firmer than impression.

Who is a good candidate

Often suitable: adults with joint or soft-tissue pain that has not responded to conservative care, who are well enough for an outpatient procedure and who understand the treatment is investigational.

Often not suitable: active infection, active malignancy, significant clotting disorders or anticoagulation that cannot safely be paused, pregnancy, or a joint that is better served by orthopedic assessment and surgical consultation. Some joints are. We would rather refer you than take your money.

Practical details

We are direct-pay. Investigational regenerative therapy is not covered by insurance. Pricing is given in writing before anything is scheduled, and nothing begins until you know the total. Many patients use HSA or FSA funds — check your plan, as eligibility varies.

Questions we get asked

Is this the same as a stem cell injection? No. Nothing in this preparation contains living cells from a donor. The structural material is your own blood.

Is it FDA approved? No. It is investigational, as stated at the top of this page. Any clinic telling you otherwise is not describing it accurately.

How many sessions? That depends on the joint and the findings. We will give you a number before you start rather than after.

Does it hurt? There is discomfort during and after, as with any joint injection. The research above suggests the ozone component reduces the post-injection soreness compared with a plasma preparation alone.

How soon would I know? Reassessment is typically at six to twelve weeks, which is the window where the published outcome data is strongest.
